Job Description:

Hopewell Horizons Home Care is seeking compassionate and dedicated individuals to join our team as Home Health Aides. As a Home Health Aide, you will play a vital role in providing essential care and support to individuals in the comfort of their own homes.

Responsibilities:

Personal Care Assistance: Provide assistance with activities of daily living, including bathing, grooming, dressing, toileting, and mobility assistance.

Medication Reminders: Assist clients with medication reminders as prescribed by healthcare professionals.

Meal Preparation: Prepare nutritious meals and snacks according to dietary preferences and restrictions.

Light Housekeeping: Perform light housekeeping tasks such as dusting, vacuuming, laundry, and maintaining a clean and safe environment for clients.

Companionship: Engage clients in meaningful conversations, recreational activities, and companionship to promote socialization and emotional well-being.

Transportation: Accompany clients to medical appointments, errands, and outings as needed.

Observation and Reporting: Monitor and document client's condition, changes in behavior, and any concerns, and report them to the appropriate supervisor.

Respect and Dignity: Treat clients with respect, dignity, and empathy, honoring their preferences, beliefs, and privacy at all times.

Collaboration: Communicate effectively with clients, families, and healthcare team members to ensure coordinated and holistic care.

Adherence to Policies and Procedures: Follow agency policies, procedures, and protocols to ensure the safety, well-being, and rights of clients are upheld.

Requirements:

High school diploma or equivalent.

Successful completion of a state-approved Home Health Aide training program.

Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is preferred but not required

Demonstrated experience in providing personal care assistance to individuals in a home care setting.

Strong interpersonal and communication skills.

Compassionate, patient, and reliable, with a genuine desire to help others.

Ability to work independently and as part of a team.

Valid driver's license and reliable transportation (if required for the position).
