1-dot card
2-valid drivers license
3-16ft box truck
4- valid insurance 300,000 combined
5- your name must be on the insurance declaration page listed as a covered driver
6-clean motor vehicle record no speeding tickets over 15 miles an hour within the last three years
7- must be able to pick up 100 pounds
